Print & Fill Out Forms
You can complete the patient forms at home or complete them in the office prior to your visit.

Bring Insurance & ID
Please remember to bring a form of ID and your insurance card with you to your office visit.

Testing Information
We are pleased to provide the following tests and evaluations:
- Hearing Tests and Evaluations
- Speech test: Your hearing care professional will test your ability to hear and repeat words spoken at different sound thresholds. This is sometimes done in conjunction with background noise.
- Uncomfortable Loudness testing ( This is sometimes skipped by other offices, but very important)
- Hearing Aid Evaluation
